PHYSICAL ACKNOWLEDGMENT
A current IHSA physical form, signed by a licensed physician, physician’s assistant, or nurse practitioner must be on file at the school to practice or participate in athletics. The physical examination is valid for 365 days from the date of the exam.

D214 CO-CURRICULAR CODE OF CONDUCT
The Athletic Programs provide a wide variety of sports to match a wide spectrum of talents and interests and to develop among students fitness, skills, sportsmanship, and a positive spirit of competition.
A student must have his/her parent’s or guardian’s and physician's permission to participate in the athletic program. The athlete must abide by the IHSA eligibility by-laws. To ensure the proper atmosphere for athletic excellence, the athlete and his/her parent(s) or guardian(s) must understand and cooperate in helping establish that atmosphere by adhering to all school rules and regulations. An athlete may be suspended or dropped from a team for the remainder of the season for violating any of the following standards: (1) falsification of physician’s signature on the athletic permit; (2) use, possession, or distribution of alcohol or tobacco or the abuse of any controlled substance; (3) theft or destruction of property; (4) repeated activities. Finally, all athletes are subject to the provisions of the District 214 Co-Curricular Code. Once a student becomes a member of an athletic team, the athlete cannot quit and become a member of another team in the same season. Athletes who are cut from a team are encouraged to try out for another team.

IHSA STEROID TESTING POLICY - CONSENT TO RANDOM TESTING
Any student-athlete who ingests or otherwise uses a substance listed in the association’s banned drug classes, without the written permission of a licensed physician, to treat a medical condition, violates IHSA By-law 2.170 and its subsections and is subject to IHSA penalties, including ineligibility from competition. Accordingly, the IHSA will test certain randomly selected individuals and teams that participate in state series competitions for banned substances. The results of all tests shall be considered confidential and shall only be disclosed to the student, his/her parents, and his/her school. By signing below, we consent to random testing in accordance with the IHSA’s steroid testing policy. We understand that, if the student or the student’s team participates in state series competitions, the student may be subject to testing for banned substances. No student-athlete may participate in the IHSA state series competition unless the student and the student’s parent/guardian consent to random testing.

IHSA HATE SPEECH AND HARASSMENT POLICY
- If contest officials witness hate speech or an act of harassment, it shall be penalized by an IMMEDIATE EJECTION.
- If contest officials do not witness hate speech or an act of harassment, the target will inform their coach of what happened.
- Coach tells the official and play stops. Teams return to their benches. The official meets with the alleged offender and the opposing coach.
- If admittance, ejection from contest, suspension for next contest and subject to school discipline for hate speech. If the official hears the use of hate speech, they must submit written documentation of what they heard to be included in the IHSA special report.
- If not admittance, remind the player of the consequences.
- The target is informed and told to continue telling the coach if the offense continues. The coach continues to inform the official and opposing coach during a natural break in play.
- Coaches inform the AD’s within 24 hours of the incident. The AD meets with any athlete identified as possibly using hate speech or alleging the use of hate speech, and the other AD meets with the target.
- If nothing is said until after the contest, the coach informs the AD who relays information to the opposing AD. Both ADs meet with participants and communicate their findings back to the other AD.
The ADs inform the IHSA Administrator of the incident and document the incident using the IHSA Special Report for Hate Speech and Harassment option.

INFORMED CONSENT
Participation in athletics includes the potential for injury. We realize this and acknowledge that even with the best coaching, the use of the most advanced and safe equipment, and the strict observance of rules, injuries are possible. We also acknowledge that on rare occasions, injuries can be so severe that they result in permanent disability or death.

SWAY MEDICAL - CONSENT TO PARTICIPATE
District 214 uses Sway Medical to monitor sports-related concussions and to ensure recovery and prevent recurrent, cumulative, and/or catastrophic consequences from a second concussive injury. By signing below, parent/guardian consent is given for students to participate in this service, and the parent/guardian acknowledges that they have read the IHSA concussion policy information.

MEDICAL/EMERGENCY RELEASE
By signing this form, you authorize the school to take such emergency action that may be necessary, including transportation of the student to a hospital or medical center, if neither parent nor the emergency contact can be reached in case of severe injury or illness.

ACCIDENT INSURANCE
High School District 214 has partnered with 1st Agency to provide parents/guardians with the opportunity to purchase low-cost student accident insurance for their children while at school, during football/athletics, and during co-curricular activities. PARENT/GUARDIAN/FAN D214 CODE OF CONDUCT
The IL State Board of Education and IHSA require school districts to monitor spectator behavior at all school sponsored activities. To ensure a positive and enjoyable environment for all, we expect all parents, guardians, and fans to adhere to the following code of conduct:
- Respect all Participants: Show respect to all athletes, coaches, officials, and other fans. This includes refraining from verbal abuse, harassment, or intimidation.
- Positive Sportsmanship: Display good sportsmanship at all times. Cheer for your team and avoid negative comments or actions towards the opposing team or officials.
- Follow the Rules: Abide by the rules and regulations of the school and IHSA. This includes following all venue policies and guidelines.
- Support the Coaches: Support the decisions of the coaches and officials. Remember that they are working to provide a safe and fair environment for all athletes.
- Be a Role Model: Serve as a positive role model for young athletes. Demonstrate good character and sportsmanship through your words and actions.
- No Hate Speech or Harassment: Any form of hate speech, harassment, or discrimination will not be tolerated and will cause immediate ejection and potential disciplinary action.
If the code of conduct is violated, a building administrator or supervisor may request any person involved to leave the premises and may contact the police for assistance. The Board of Education and their designees have the authority to suspend individuals from attending all school-sponsored activities (home and away) for up to one year for spectator conduct.

REQUIRED SIGNATURES OF AGREEMENT PARENT/GUARDIAN
By his/her signature, the student-athlete agrees to abide by all of the conditions set out by District 214 and the IHSA. By his/her signature, the parent/guardian of the student-athlete gives his/her permission for the student-athlete to participate in interscholastic athletics under all of the conditions described above.
Online Registration

We are now offering a new online registration program for interscholastic athletics through Infinite Campus. This will be the only way to register for participation in athletics and must be completed by a parent/guardian. Students cannot complete the registration.
