1. Applications for a work permit can be downloaded or picked up in Student Services Office.
2. Student completes the top portion of the form.  Employer completes Statement section.
3. During the school year, the Statement of Physical Fitness is signed by the School Nurse.
   During the summer, parents must have the Statement of Physical Fitness signed by their doctor.  
4. Parents sign application in-person in the Student Services Office.
5. Certificate of Employment is filed with the Illinois Department of Labor by the Student Services Office .
